Copy of child's birth certificate

Child's most recent half-year or end-of-year report

Copies of remedial/psychological or educational reports or assessments

School fee account statement for the previous and current year

Proof of payment of the non-refundable R500 application fee into the account shown below

Both parents/guardians must sign the application
